The advance directives document

An advance directives document records the instructions and guidelines we want healthcare staff to follow regarding the treatment and care we do or do not wish to receive. It takes effect when we no longer have full capacity to express those wishes ourselves.

That is why it is important to formalise this document and register it in the Register of Advance Directives kept by the Catalan Department of Health.
